There is a $250 deductible per claim.

What do I do if I am injured while participating in intercollegiate athletics?
All injured athletes must report to the certified athletic trainer who is responsible for their team or to the head athletic trainer as soon as possible. At that time a thorough evaluation will be performed to determine the injury and if referral to other medical personnel is necessary. If you are referred to a physician or the emergency room you must fill out a secondary insurance form and bring it with you to have the physician fill out a portion, you must then have your parents or guardian fill out a portion and then the athletic trainer will fill out the final portion and send it to the secondary insurance carrier. All bills must first be sent to your primary insurance carrier, then any outstanding bills, along with the primary insurance Explanation of Benefits (EOB) must be sent to the secondary insurance carrier. Failure to follow these steps may result in slower or no payment from the secondary insurance carrier.
What is the physical/athletic insurance packet?
Each year every student athlete must complete all forms before participation in an official practice or play. All freshman and transfer students must also have a physical exam from a licensed physician prior to participation in any intercollegiate sport, including official practice. The pre-participation health history questionnaire must be updated yearly and should include any injuries or illnesses you incurred during the past year even if they were evaluated and treated by the athletic training staff. A copy of the physical/insurance packet can be found here.
